Has anyone had trouble with the exterior, manual mirrors being loose? Every time one shuts the door, the mirror has to be readjusted. Is there any way to tighten them?

Most mirrors do have an adjustment screw on the nuckle.

It is the actual glass inside the plastic that is adjustable and there does not appear to be a screw anywhere. Even shining a light down in behind the mirror glass does not reveal anything.
